SUU PREP New Student Application

Dear New SUU PREP Applicant,

Thank you for choosing to apply for SUU PREP. We know that if you are selected for our program you will learn, grow, be challenged, and have fun! Please be aware of a few important items before you apply:

  • We have a very strict Student Code of Conduct, as well as a strict Attendance Policy. Please read this information on our home page before applying, and be aware that any violation of these policies are grounds for dismissal from SUU PREP.
  • SUU PREP is designed to challenge you. We know you can succeed, but be prepared to push through hard concepts, keep a positive attitude, and know that it is all worth it in the end! We have rigorous classes, so be prepared to learn! You must have an 86% or above in your math class to apply.
  • Items needed for application:
    1. SSID Number: State Student Identification Number
      This is NOT your lunch number, nor is it on PowerSchool. This is the number you use for you state school tests. It is 7 digits. Your SSID number must be obtained by having your parent/guardian call your school’s registrar secretary. This is a state issued number that we use to track student’s progress and school changes. Please see the research release forms for more information or contact suuprep@suu.edu with questions.
    2. Screenshot or scan of most recent end of quarter grades. Students will not be accepted with any Ds, Fs, Ns, or Us. Applications are not first come, first serve (they are graded competitively, based on a rubric) so if you need to improve your grades and submit your application at the end of this current quarter, please do so. Applications close February 1st.
    3. Correct email address of a teacher you would like us to send your teacher recommendation form to. We highly suggest having a math teacher as your teacher contact.
    4. You will be asked to write an essay (150 words minimum) to the prompt "Why I Want To Attend SUU PREP". We suggest writing your essay and revising it through Word, Google Docs, or another applications and then copying and pasting into our form. This way you have time to have a thoughtful answer, and edit! These essays are a heavily weighted item on our acceptance rubric.
    5. Your parent/guardian available. They will need to sign a few forms.
